HIV Positive Registration Form
If you are HIV Positive and would like to register for the walk, please use our
Complimentary Registration Form.
Complete the form and fax or mail to the address at the bottom of the form. The $25 registration fee is waived for all HIV Positive walkers.

PLEASE NOTE - IN ORDER TO GUARANTEE YOUR SELECTED T-SHIRT SIZE, ALL PAPER REGISTRATION FORMS MUST BE RECEIVED BY AUGUST 10, 2009.
