HIV Positive Registration Form
If you are HIV Positive and would like to register for the walk, please use our
Complimentary Registration Form.
Complete the form and fax or mail to the address at the bottom of the form. The $25 registration fee is waived for all HIV Positive walkers.

Extra pledges form - Please use this form to obtain additional off-line donations for the walk.
Prizes will be awarded to the walker with the highest dollar amount of donations.
